General instructions (reference)
This log book is intended to be a record of all the activities of Postgraduate students, as they perform and participate in the course, including training. Each resident has to submit the Log Book before they appear for the Final examination. Otherwise, he/she will not be allowed to appear in the examination.
- The log book must be filled on a regular basis. It must be signed by the faculty in-charge & HOD within one week of the last day of posting.
- Marks entered are the average marks based on collective evaluation of faculty, entered and signed by the faculty moderator.
- Faculty should record specific comments in the remarks column where relevant.
- In the seminar/JR sections, faculty must comment on the level of participation and interjection by the student.
- Evaluation of a seminar must include the write-up submitted by the student.
- For laboratory postings: mention only the total number of samples processed, along with interesting findings and any notable patient/case history.
- All trainings/CME/conferences attended must be supplemented with certificates.
- All end-of-posting exam evaluations are entered as a percentage.
- Attendance at MBBS lectures is mandatory, but only PG-conducted teaching sessions are entered in the log book.
- Extrinsic and intrinsic (Pathology & Biochemistry) posting records must be signed by the in-charge/HOD of the respective department/institute.
- It is the student's responsibility to ensure entries are made day-to-day and relevant documents are kept.
- Periodic evaluation of log book maintenance is carried out by the Postgraduate education office.
- Corrections can be made under instruction from the Head of Department. Additional pages can be added if required.

Essential pre-requisites for appearing for examination
- Log book of work done during the training period, including rotation postings, departmental presentations, and internal assessment reports, must be submitted.
- At least two presentations at a national-level conference. One research paper should be published/accepted in an indexed journal.
The postgraduate examination has three parts: Thesis, Theory examination, and Practical/Clinical & Oral/Viva Voce examination — conducted per the latest Postgraduate Medical Education Regulations.
